Although we are willing to assist you, it is ultimately the patient's responsibility to obtain referrals and authorizations. If you are not sure whether you need a referral, please check with your Primary Care Provider or insurance carrier.
Co-pays are to be paid at the time you are seen. The patient is also responsible for any deductibles. We will try to work with you in the case of financial hardship.
